(4) H.U.G. Reading Program's Code of Ethics
The mission of the H.U.G. Reading Program is to promote the literacy skills of struggling readers in elementary schools.
All volunteers must adhere to the general rules while in service for the children during the H.U.G. Reading Program.

The following specific rules of conduct guide volunteer behavior and are not intended to limit the application of H.U.G. general rules. The guidance below is developed to protect each group: students, volunteers, specialists and tutors.

H.U.G. Volunteers will treat all students' participants, parents, other volunteers and staff with respect and consideration. H.U.G. will not discriminate against anyone on the basis of age, race, gender, culture, socioeconomic status, religion/creed or handicapping condition.

H.U.G. Volunteers will use only positive techniques while working with the students including: positive reinforcement, encouragement, redirection and anticipation and presentation of potential problems. Volunteers will alert school staff to any issues shared about a harmful situation in the child's life.

H.U.G. Volunteers should be at their assigned Zoom room during regularly scheduled tutoring sessions.

H.U.G. Volunteers' clothing must be appropriate.

H.U.G. Volunteers will not use profanity in the presence of the students.

H.U.G. Volunteers will maintain confidentiality and will discuss information about students only with the H.U.G. Specialist. The online website access will have pass code protection for all student information.

Falsifying information on the H.U.G. application is a breach of the Ethics code and will result in the applicant being denied the opportunity to volunteer with the H.U.G. program.

H.U.G. volunteers should report, in confidence, any suspected violation of the Ethics Code to the H.U.G. Specialist.

NO private gifts or food should be given to the students. ALL students will be given a summer reading book and the volunteers can write messages in the books.

Contacting students outside of H.U.G. sessions (physically or digitally) is not recommended. This is to protect the volunteers and the student to maintain the best relationship for tutoring.
(5) Video/Photograph Release
I hereby grant H.U.G. Reading Program (the “Company”) the irrevocable right and permission to use any photographs and/or video or audio recordings of me on their website and other websites or for any other promotional purposes without compensation to me.
I understand and agree that such photographs and/or video recordings of me may be placed on the Internet. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video or audio recordings of me. I waive the right to approve the final product or any version of the product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of the Company.
I hereby release, acquit and forever discharge H.U.G. Reading Program, the Company, its current and former trustees, agents, officers and employees of the above-named entities from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said photographs and/or video or audio recordings, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.
I hereby warrant that I am eighteen (18) years of age or older and competent to contract in my own name or, if I am less than eighteen years of age, that my parent or guardian has signed this release form below. This release is binding on me, my heirs, assigns and personal representatives.
(6) H.U.G. Reading Program Waiver
I understand that as a volunteer tutor for the H.U.G. Reading Program, I will be providing tutoring services to participants in the program. I acknowledge that my participation is voluntary and that I will not receive financial compensation.
**Assumption of Risk:** I recognize that there are certain inherent risks associated with volunteering as a tutor, and I assume full responsibility for personal injury to myself and further release and discharge the H.U.G. Reading Program for injury, loss, or damage arising out of my use of or presence upon the facilities of the H.U.G. Reading Program, whether caused by the fault of myself, the H.U.G. Reading Program, or other third parties.
**Release and Waiver:** I release, waive, discharge, and covenant not to sue the H.U.G. Reading Program, its directors, officers, employees, volunteers, agents, and successors from any and all liability, claims, demands, actions, and causes of action whatsoever directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by me related to tutoring services while participating in any activity or being on the premises involved with the H.U.G. Reading Program.
**Indemnification:** I agree to indemnify and hold harmless the H.U.G. Reading Program from any loss, liability, damage, or costs, including court costs and attorney's fees, that they may incur due to my participation in the program, whether caused by negligence of the program or otherwise.
**Acknowledgment of Understanding:** I have read this waiver of liability, assumption of risk, and indemnity agreement, and I fully understand its terms. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
**Governing Law:** This Waiver and Release Form shall be construed in accordance with, and governed by, the laws of the State/Country in which the H.U.G. Reading Program operates, and any legal action relating to or arising out of this Agreement shall be commenced exclusively in the state or federal courts located in [Jurisdiction].
I HAVE READ THE ABOVE WAIVER AND RELEASE, AND BY SIGNING IT, AGREE IT IS MY INTENTION TO EXEMPT AND RELIEVE THE H.U.G. READING PROGRAM FROM LIABILITY FOR PERSONAL INJURY, PROPERTY DAMAGE, OR WRONGFUL DEATH CAUSED BY NEGLIGENCE OR ANY OTHER CAUSE.
